Subject: – POLIO CAMPAIGN 20^th Sep to 26^th Sep, 2021

1. As per directive of the Ministry of Health & District Emergency Operation Center (DEOC), the Polio Campaign has been scheduled to vaccinate all the children under 5 years’ of age from 20^th Sep to 26^th Sep, 2021.

2. In this regard, kindly intimate whether your child to be administered with polio vaccine or not by ticking appropriate box. Yes                                 No

3. The above information to be provided to respective Class Teacher latest by Saturday 18^th & Monday 20^th September, 2021 Your cooperation will be highly appreciated, please.
