Subject: – POLIO & VIT-A VACCINATION Monday – 2^nd Oct, 2023

 

 

1. As per directive of the Ministry of Health & District Emergency Operation Center (DEOC), the Polio Campaign

has been scheduled to vaccinate all the children under 5 years’ of age on Monday , 2^nd Oct, 2023.

 

2. In this regard, kindly intimate whether your child to be administered with Polio & Vit-A vaccine or not by ticking

appropriate box.
