1. As per directive of the Ministry of Health & District Emergency Operation Center (DEOC), the Polio
Campaign has been scheduled to vaccinate all the children under 5 years’ of age from 15 th Aug to 21 st
Aug, 2022.
2. In this regard, kindly intimate whether your child to be administered with polio vaccine or not by
ticking appropriate box. Yes No
3. The above information to be provided to respective Class Teacher latest by Wednesday 17 th
August, 2022 positively. Your cooperation will be highly appreciated, please.
