1. As per directive of the Ministry of Health & District Emergency Operation Center (DEOC), the Polio Campaign has been
scheduled to vaccinate all the children under 5 years’ of age on Tuesday, 14 th March, 2023
2. In this regard, kindly intimate whether your child to be administered with polio vaccine or not by ticking appropriate box.
